Synovial lactic acid and septic arthritis.

چکیده
To the Editor: In their Rational Clinical Examination article, Dr Margaretten and colleagues concluded that the synovial fluid white blood cell count and the corresponding percentage of polymorphonuclear cells are the most useful markers in identifying septic arthritis while waiting for the Gram stain and culture test results. Synovial fluid glucose, protein, and lactate dehydrogenase were not found to be helpful. In the past, synovial fluid lactic acid measurement was proposed as a useful test in the rapid differentiation between septic and nonseptic arthritis. For example, in the study by Riordan et al, lactic acid seemed to be more sensitive than Gram stain (especially if antibiotics had been administered before joint aspiration) and could be measured even when the synovial fluid was too thick for a cell count to be performed. In addition, synovial fluid lactic acid can be assessed rapidly in a blood gas analyzer and may be available to clinicians even before the synovial fluid cell count and differential. It would be helpful if the authors’ literature search provided data on the value of this easy-toobtain and seemingly useful test.

Lactate UV-system: a rapid method for diagnosis of septic arthritis.
The concentration of lactic acid in synovial fluid was estimated in 43 specimens from patients with an acute monoarthritis by a simple enzyme method. In 9 patients with 10 episodes of septic arthritis concentrations of synovial fluid lactic acid were significantly higher (mean 10.8 mmol/l) than in 33 patients with nonseptic effusions (mean 3.1 mmol/l). With this method concentrations of synovia...

Background: Triggering receptors expressed on myeloid cells i.e soluble triggering expressed on myeloid cells-1 (STREM-1) is inducible on monocyte/macrophages and neutrophils and accelerates tissue destruction by propagating inflammatory responses in disease related to bacterial infections. The aim of this study was to investigate the role of STREM-1 in the synovial fluid to identify septic...
متن کاملGas-liquid chromatographic analysis of synovial fluid: volatile short-chain fatty acids in septic arthritis.
Volatile short-chain fatty acids (SCFA) in synovial fluid from 80 patients were quantified by gas-liquid chromatography (GLC). Characteristic patterns of volatile SCFA could not be associated with septic, nonseptic inflammatory, or noninflammatory groups. Mean concentrations of pentanoic and hexanoic acids were similar in all groups studied. In the septic arthritis group 3 of 4 patients with ac...

Background: Diagnosing the etiologic causes for septic arthritis is very important. The main goal of study was to determine the group A streptococcal polysaccharide antigens in synovial fluid of patients with arthritis. Methods: A cross sectional study was performed upon 52 cases with acute mono arthritis in Hazrat-e-Rasool hospital in Tehran Iran (2010-2012). Techniques used were: Gr...
متن کاملSerum procalcitonin for discrimination between septic and non-septic arthritis.
BACKGROUND Early differentiation between septic and non-septic arthritis is difficult. A previous study showed promising diagnostic accuracy of serum Procalcitonin (PCT) in septic arthritis, limited by a low sensitive PCT test kit. OBJECTIVE To investigate the diagnostic value of PCT in patients with septic and non-septic arthritis using a novel test with low detection limit. METHODS Forty-...
